I think you should tellher to consult a medical professional. this isn't the place to ask for medical advice, or to discuss an eating disorder. These are boards for women TTC

Sadly eating disorders can affect your hormones and therefore your cycle. Having her disorder well in hand is the best way to help regulate her cycles and give her the best shot TTC. However, this is just a guess based on little knowledge. If you knew her specific concerns we might be of more help. Also just an extra aside, she should definately feel confident in her mental state before TTC because post partum depression, sleep deprivation, feeling overwhelmed, physical demands etc. Are a real thing one the baby come and she wants to go into things with the best possible start.

Anorexia and being underweight can cause you to stop ovulating. In addition, you need to eat especially when pregnant. For a woman of average weight it is recommended that you gain 25 to 35 pounds during pregnancy. This isn't all fat of course, it's the baby, placenta, amniotic fluid, increased blood, etc. so she should be prepared for the weight gain. After delivery, your body is obviously different which can be especially hard for someone who already has body image issues. I'm sure this sounds like obvious importation but things your cousin should prepare herself for. It takes a lot of courage to seek help and hopefully she can be proud of herself for overcoming this very common, but not widely discussed issue.

Short answer-- no it's not easy but it is possible

Long answer- One of the symptoms of Anorexia Nervosa(AN) is amenorrhea (i.e. lack of periods), this one of the core features to diagnose AN. However, many people with AN move in and out of the core diagnostic group over time (i.e. may not always tick all the boxes to meet AN but have disordered eating and related psychological distress nonetheless). It is also possible that you may have AN and have disrupted cycles but not complete amenorrhea. Without knowing much more physical detail about the individual from detailed physiological investigations it would be difficult to say how "easy" it will be for any given individual. I would also tend to agree with others that the physical side of preparing for pregnancy is only one aspect to this situation and addressing the psychological impacts of pregnancy and impending motherhood are also really important.
