Low Testosterone

DH was diagnosed with an extremely low sperm count, had blood work done and we've found he has low Free Testosterone. Have any of you had this issue? If so what was recommended and did DHs count increase?

Hi! Yes, we have the same issue. My DH was diagnosed with azoospermia (zero sperm) back in May. We knew before his SA that he had low T, although we still dont know why. It's been a LONG road but this is what we've found out so far...
All of DH's hormone levels were low (T, FSH, LH). He was receiving testosterone injections to help with his low T and that actually causes men to have a zero sperm count. (A lot of physicians dont even know this. The one that prescribed it knew we were TTC) It essentially works the same as anabolic steriods. Our RE put Dh on HCG injections- 2000 iu 2x's/week and clomid 50mg/day. He was on that for 6 weeks (I believe) and we repeated his hormone levels and SA. His hormones had improved some but his SA was still zero sperm. Our RE felt like he was responding slowly so he increased his dose of HCG to 2000iu/3 times a week. He repeated his bloodwork and SA again 6 weeks later and his hormone level was much better but not quite normal. Most importantly...we had SPERM!!!! Now it wasnt a lot of sperm but it was still some. We were super excited bc I was worried we might not get any at all. RE told us to continue the meds for another 6 weeks and retest. He is due to go again next week. In the meantime we are continuing to "try" naturally.

Testosterone is important but it's not the most important. Did they test your dh's FSH and LH levels?

Male factor infertility is so difficult! I am so sorry you all are dealing with this. Please let me know if you have any questions for me or if you just need to vent. Good luck and keep me updated!

That is so awesome that the treatment is working! Hopefully his count will even be higher next time! I googled as soon as I talked to the nurse about his test results and was surprised to find that treating low T won't work with straight testosterone. I'm glad I researched it.

I know they did blood work but I'm not sure what all they tested. Our RE recommended going straight to IVF and I was so devastated by that news I couldn't think about anything else. We've got an appointment with a urologist at the end of December to go over everything and I'm going to ask about the FSH and LH also.

Did you RE give you your options for getting pregnant? I'll do IVF if we have to but would love to take the least inexpensive route. And are there any major side effects to the treatment. I'd like to warn DH ahead of time since I'm sure his treatment will be similar. Thank you so much for the information!

I agree about the IVF. We will do that if we have to but would much rather not have the stress of spending all that money. Our RE wanted to try and correct his hormone issues and see if that would result in Dh producing a "normal" amount of sperm. He has mentioned IVF but really doesnt want to discuss it unless we have to. My DH has not had any side effects that we know of. Some people say clomid can cause men (and women) to have mood swings, become difficult to deal with. I think it did do that to my Dh but it wasnt awful. I'm a nurse so the injections werent a big deal for us but that may be uncomfortable for some people.

I would definitely try and find out what they think is the cause of your dh's problem. It may not be something that can be "fixed" by using medication but I would ask. My Dh's co-worker had the same issue as we do and he took the same medication. He and his wife got pregnant immediately after his hormone levels returned to normal range.

Please let me know what the urologist says. I will keep you updated on our progress also!

Are your DH's also trying Macca, Zinc and other vits/herbs combined? What about dhea? Not sure if Dhea is part of the protocol...?

I could open a pharmacy with all of the stuff DH takes. He's on Maca and zinc plus a ton of other stuff. We've got another SA coming up to see if they're working. Fingers crossed they are!

Hi! My DH takes fertilaid but I will have to look into the other supplements...hmm. Thanks for the suggestion! We haven't gotten his results back yet. He also has to go back for bw to see where his hormone levels are on Wed bc the lab told him he didn't need bw today when he really did. Frustrating! Especially since I told him this morning to make sure he had it done and he didn't listen : ). I will keep you updated on the results! Look forward to hearing how your DH's next SA goes as well!

So we received the results of Dh's last SA...he had 2.2 mil/ml!!!! They said there were 2.2 mil mobile sperms per ml which is definitely and increase from the 100,000/ml in October. We are supposed to continue the meds for another 8 weeks and retest at that point. Our RE also told the nurse to tell us that at this point we should start "trying." He wants us to use a fertility monitor or OPKs and time our BDing well. It was so exciting to hear that we should "try" now!

Anything new with you guys?

I literally almost cried when I read this. That is such amazing news! It gives me hope :D

We have an appointment December 22nd with a urologist to hear what he has to say about his low testosterone and we've also got to have a repeat SA since he's been taking vitamins and other supplements. The doctor wanted to see if those were working. I really can't wait to see what the urologist says and hope DH can start some type of treatment soon!

Aw, thank you! I'm so glad it gave you some hope. I am trying to be as positive and hopeful as possible. I have moments where I feel like we will still end up having to do IVF but again, at lease we have sperm to do IVF with now!

I will be thinking about you lots on Dec 22nd! I hope you get some great info and that his SA is much better. Dont forget to ask about his other hormone levels. Find out what his FSH and LH are because those are very important too.

Hi girls! My DH also has low testosterone and low sperm count. He's been on Clomid for the testosterone and it HAS helped his hormone levels, but not his sperm count, unfortunately :( We are very grateful for our DD who is almost 5 now, and we've been TTC over 2 years. We were going to do IUI but his sperm count is too low for that, and IVF is not an option for us.

Our urologist is a total wierdo! We think he may have been drunk during our appointment, but despite all of that he's very knowledgeable! He put DH on Clomid and L-carnitine (basically a multivitamin) and we go back in two months to see if it's doing anything. He recommended doing surgery again to repair the varicocele that the original doctor failed to fix and he thinks that may help some. He doesn't think his count will ever be in the range for us to conceive naturally but he wants to see his count go up so he's got a good number for IVF.
