Semen analysis results and heartbroken. What's next?

Hi all,

Backstory, my DH and I have been ttc for almost 1.5 years. We did some initial testing about 6 months ago, my progesterone was a little low, but prolactin and TSH levels were normal. My doc prescribed 50mg clomid for 6 months and follow up tests confirmed ovulation and good prog levels, but no BFPs for us.

Fast forward, I went back to the doc who said next steps would be HSG for me and a semen analysis for DH. I should've waited for the SA results to come back before doing the HSG because OUCH that hurt! Everything was normal though, no blockages.

Doc calls today with SA results and DH has a low sperm count :( she said normal amount was >20 mill/mL and DH's was 4.9 mil. I don't know why I always assumed I was the problem. DH has never had any issues there whatsoever, TMI but he's gotta a boner and ready to roll anytime of day in 5 seconds! No other health issues, never even smoked pot or a cigarette. I know it's naive of me to make any assumptions and I truly don't intend to be judgmental in thinking any male is at fault for sperm issues. I guess it's just the shock of it or maybe just false hope that nothing was wrong. My doc couldn't give me any information on the outlook from here bc we will need to go see a male fertility specialist. The information on the Internet is so overwhelming and difficult to consolidate and absorb.

So ladies, can anyone give me possible outcomes here, good or bad? Are there treatment options? Does a sperm count this low mean our chances of conceiving are very slim? Or is his level low, but not low low? Of course success stories are encouraging, but I also need some brutal honesty with trying to understand. And maybe after the initial surprise wears off, I'll get to the point of feeling thankful that at least we have some answers.

My DH has similar numbers. Something like 5 million sperm and only half of them are good. I've been pregnant only through ivf. I've done three mini ivfs and have one daughter and I had a miscarriage my last attempt. However, I have one blocked tube. I would suggest supplements, exercise and a healthy diet. My DH's count went all the way up to 34 million during the cycle for dd in 2012. This last ivf attempt, his counts were super low. So I thought back to 2012 as to what might have increased his counts. He was on Procreation male fertility supplements, drinking spinach and fruit smoothies he would make himself, and he was walking miles everyday. The smoothies and walking were not done for fertility purposes though, he just happened to be into that kind of thing at the time. Hopefully you can conceive without ivf, supplements and fruits and veggies might help you too.

DH had a count of 12 million when tested 2010 when we first started investigating why I wasn't getting pregnant. He started taking multi vitamins for males to assist with conception. He also took a separate supplement for lycopene which was highly recommended to increase count. He cut down on sodas and tried to eat less processed food. 6 weeks later he had another SA and his counts were up to 25 million. The doctor was really surprised but said that the vitamins and supplements definitely helped. We are ttc no. 2 and he has gone back on the multivitamins for conception and his counts are always around 30 million. GL

Clomid unmonitored July 2011 - BFP - DS born Feb 2012
TTC no.2 since January 2013
2014 - 6 rounds clomid or femara - all BFN
Dec 2015 - iui no. 1 - injections, 1 mature follicle BFN
Jan 2016 - iui no. 2 - injection, cancelled due to overstimulation - 8 mature follicles

Thank you both so much for your reply. Did either of your DHs have any issues with motility or morphology? I got the full report and my DHs were slightly abnormal in these areas too. We have an appointment with a urologist on Monday and I'm anxious and scared to hear what they say. DH started on a vitamin right away so hopefully that helps. He exercises regularly and is underweight if anything, but there is definitely room for improvement on the quality of his diet.

My DH didn't have motility or morphology issues - just count. My DH also went to a urologist and they diagnosed varicocele but given that the sperm count increased, I don't think this was causing the problem. I really think that my husband's work situation played a part in the low count - he was going through a stressful period at work and as well as taking the lycopene and slightly changing his diet, we were lucky that the stressful period in work passed. I credit this in conjunction with the lycopene for improving his counts. Sorry, I don't know much about motility or morphology from personal experience but I have seen on board where men can be put on clomid to improve sperm.
