Upcoming Orientation - What were your biggest questions?

Hello everyone, my IVF journey is about to begin I think, and soon I'll happily (and gratefully) be joining one of these threads to share my experiences and support with you :)

I can't believe we're at this step already, but DH and I have our IVF orientation booked for next week. I am certainly not an IVF expert, and totally look forward to learning more about the process.

I'm wondering what (if you can remember) your most valuable questions and answers were to your doctors when you had your orientations. I know they always say to write your questions down so that you don't forget anything - I feel like I'm forgetting what I don't want to forget about already! And, another part of me doesn't even realize what questions I should be asking.

I'm one of those people where, the more information I can get, the better off I am.

Welcome to the IVF board!

Some questions I thought were helpful to have answered were:

* Do they do plan to do ICSI or natural fertilization? Some try all natural, some 50/50, some all ICSI.

* Do they prefer 3 day transfers or 5 day transfers? How many good quality embryos do you need by day 2 or 3 to have day 5 be an option?

* Will you be "put under" during the egg retrieval?

* What steps do they recommend for preventing OHSS? Generally it's gatorade and lots of protein, but some doctors are pro-salt while others are anti-salt.

* How many embryos or blastocysts do they recommend transferring for you specifically?

* Do they recommend bed rest after the embryo transfer?


I bet you'll realize what questions you may have while they're walking you through everything at orientation, but like you I wanted to be 100% prepared. :)

My biggest question was what are my chances haha. But for you, I think your chances are good since you are 31. About one in two of your eggs will be chromosomally normal on the right protocol. Is your FSH consistently 12 or is this a one time reading? My highest was 11.1 and I got a 10.4 another cycle but they were mainly high 7s and low 8s. What is your AMH if you mind me asking? Even so, a high FSH just means they will likely start you on a higher dose of FSH product, eg Gonal F. I do recommend that you go for either Gonal F or Follistim and not Bravelle. They will probably put you on Menopur as well. What clinic will you be going to? VFC?

There are so many questions but cbusch provided excellent questions. I think you can ask more questions as you go along as well. This forum is a great resource and I'm happy to help if I can. I've been through the process 4 times ugh. Good luck!

Super great responses, thank you! I'm going to write these down, along with my own questions to make sure I don't forget.

Adventurer - my AMH was 1.63. They said I'm smack-dab in the middle, but my FSH has always been pretty ugly considering my age. I don't think I've been higher than a 12, but usually pretty high. DH and I are using Olive Fertility Clinic in Vancouver - we actually live in a biggish town about four hours away. There's a sattelite clinic though that is 2 hours away where we will be monitored, and then head to Vancouver for the retrieval and transfer.

I haven't yet had my orientation (on Tuesday, ahhhhh!) so I'm not sure about anything and really worried that I don't know enough about the process! My one big question is about protocols. How do they determine which protocol to put you on? Do you have a suggestion for a link or reference where I can read up on this? And also, what is the reason that you would not recommend braville (again, I'm just totally ignorant on these issues so the more info the better!)

Hi queeny:

I read about the cancellation of your HSG. Sorry to hear that but happy to hear that everything else is going ahead!

A quick question: Is your AMH 1.6 ng/ml or pmol/L? One is in American units and the other Canadian units. The protocol is usually determined by your age, FSH and AMH. But REs can differ in their approach so I can't predict what protocol you will on. There is a long lupron (agonist) protocol where the lupron is started a few days before you actually start stims whereas the short antagonist protocol has you starting stims on day 3 of your cycle. Dr. Geoffrey Sher from Las Vegas has a great website with TONS of info on all things IVF. It is www.haveababy.com He is a big proponent is his own agonist/antagonist conversion protocol (A/ACP) for women with diminished ovarian reserve. This is a mixture of a long and short protocol combined. Not many other REs use this protocol. If you are interested in bringing it up with your RE, you can PM me and I'll give you the details. FYI, my current pregnancy is a result of his A/ACP protocol.

I advised against using Bravelle if possible because its potency can vary from lot to lot. It's derived from the urine of postmenopausal women. Products like Gonal-F and Follistim are not from postmenopausal women and are made de novo (from scratch) and therefore their potency is higher (more effective) and more consistent. Unfortunately, Gonal-F and Follistim are more expensive as well.

Adventurer, you have been so helpful! Unfortunately the orientation didn't go as well as I had hoped. We are on hold - IVF is postponed until I can get this damn cyst (or whatever it is) under control.

I have my followup mid-March. We did continue with the orientation though, but I will have another followup with the nurses and doc in March to make sure our plan is a go. So far it looks like I'm on mega high antagonist protocol with injections am and pm. And yes, that Bravelle is what I am starting on (I cringed once I found out that was part of my plan, but I'm willing to try!). I am crossing everything that can be crossed that these meds work exactly the way they should, but if not, I will look at this cycle as an experiment/learning experience since I have never taken any fertility medication ever so I have no clue how I will respond.

The website you recommended was great - I love the doctors blogs!

So unfortunately I am on hold - guess that will give me more time to research, learn and save my pennies!
