Surprise! Twins at 21 weeks

So I went in for my anatomy scan yesterday - my first ultrasound - and found out that we're having twins! My DH and I are so shocked. Twins don't run in either of our families and we were using no fertility treatments. I still feel like this might be a dream and the news hasn't completely settled in yet. Crazy!

We're having two little girls and they look good and measuring right at 21 weeks, which is where I am in this pregnancy. They are in two sacs, but the technician couldn't tell if there was one placenta or two. However, we now need to change all of our prenatal care and plans for birth since we were using a midwife and planning a homebirth. So I'm feeling really clueless and like I'm almost starting from scratch with this pregnancy, but with less time to figure everything out!

Anyway, I just wanted to share my news and introduce myself since I know I will be around here more often. :)

Wow! 21 weeks? Did you have any idea before then that you had twins baking? Congratulations and welcome to the multiples board! This is a great place to find support and ask all of your questions. I don't know that I could have made it through my twin pregnancy without these amazing ladies and all of their support and knowledge. They rock!

Congrats! Can't imagine not knowing for that long (we did IVF so we saw our guys at 6weeks and then about a hundred times after!)

Thanks everyone. We really had no idea that I was pregnant with twins. This is my first pregnancy and everything was looking completely normal so our midwife was very hands-off. This was our first u/s because we opted out of the 12-week NT scan. We did make some jokes about the possibility of twins, but never actually believed it could be true. Ha!

I will say that knowing that we have twins makes a lot of things make sense now, like why I started to show so early and the amount/type of movement that I'm feeling. The girls are side by side in the breech position, so their two little heads are next to each other just below my belly button. I posted a few (not great quality) pics from our scan below.

My midwife talked me through the report from the u/s and we have modi twins. I've been doing a ton of reading about twins over the past couple of days and I think I have a handle on what modi means. Right now our #1 priority is finding a new OB and transferring my care. Should I be looking for a MFM too? Any other tips from other modi moms?

Wow, Congratulations!! What an amazing surprise! To answer your MFM question, most OBs have an agreement or work with a MFM specialist so when you find the OB, they'll just automatically refer you to their cooperative MFM specialist. It's taken me a while to figure out exactly how they work together but basically, the OB handles all the routine, regular care and is really in charge of making the final call but the MFM specialist is there to go over all the higher risk parts of twins and monitor the babies growth, etc and report it all back to the OB. Keep us posted on how you're doing! :D

Congratulation on your twin pregnancy!!

I have never hear the term modi twins, and have to assume she means mono/di twins, which means they share the same sac/placenta. If you said di/di twins, then that would mean the babies each have a sac and placenta. I would confirm this with an OB or more importantly an MFM/Perinatologist.

My first set of twins were mono/di twins and those types of twinning is at risk for TTTS (twin-to-twin transfusion syndrome). This is when one baby gets too much blood sent to one baby and not enough to the other. There are 5 stages for this syndrome, named after the founder/pioneer Dr. Quintero, he now practices in Miami. since you are already 21+ weeks, I would HIGHLY recommend getting in to see a high risk OB IMMEDIATELY. But, have some reassurance that the u/s that you did get was recent and if all looks good, that is a great sign, but you need frequent monitoring at this point, every 2 weeks.

My boys were dx with Stage I TTTS at 22+ weeks and I went on voluntary bedrest and extra protein drinks. This must have reversed the diagnosis because 3 days after the initial diagnosis, their situation improved and I did not qualify for laser surgery but did see the specialist (Dr. Quintero) every Monday for 10 weeks. My boys were also my very first pregnancy and it can be a scary time since its your first and they are identical. (from what it looks like). My boys were born healthy at 36 weeks, no NICU. I also had a singleton daughter after them, she is now 2.5 yo and a second set of twins, fraternal g/b born full term, almost 39 weeks. I am modi also. You need to get ultrasounds every 2 weeks from a mfm. Ttts is the biggest concern, but the chance of getting it is only 15% and then if you do happen to get it only 1/4 of people in stage 1 will progress to stage 2 and require surgery. That is because of technology and good monitoring. If you aren't monitored properly then stage 1 ttts can be really really bad because it can go downhill quickly.

Now that's the bad news, the good news is that the majority of modi twins don't get ttts and those that do get it usually survive so don't be afraid of modi twins, just make sure you are monitored. And stay off google. I made the mistake of googling the heck out of modi twins and got scared. Thats when I started reading actual research articles and felt a LOT better. You also need to plan on delivering before 37 weeks. Its not very safe for modi twins to be left in past 37 weeks cause the risk of acute ttts goes up.

What helps keep my mind off the risk part of my pregnancy is to buy baby clothes. You should see my newborn stash. Lol. I'm soooo excited to have another modi mom in the Oct group with me. This pregnancy gets lonely because most twins are didi. It will be fun to follow each others pregnancies, I don't post much on Oct board anymore because the pregnancies there are nothing like mine and I can't relate to much that the other women are going through. My good news is equal fluid levels at an ultrasound and no one else gets how important that is in a modi pregnancy. I have researched the crap out of this pregnancy and I can answer most questions you have. I will also post a link to a modi fb group. There is one fb group that I had to leave because it made me scared to death and I felt like I was destined to lose my babies. Another fb group was launched today that was created by the doctors at the fetal health foundation. Ill post that one so you don't get scared to death by false info.

Wow Congrats! With modi I would definitely go for extra monitoring since they share a sac. You could have some complications towards the end, but maybe not ;) Very cool to find out your having identical twin girls after 21 weeks, crazy! It'll all work out, Congrats and Welcome!

Jade - yes, what my midwife calls modi is the same as mono/di. One placenta, two sacs.

Megan - thanks for all the info. I'm excited that our EDDs are close, it will nice to have someone to check in with over the next months! I completely know what you mean about buying baby clothes. I went a little crazy this past weekend working on our baby registries, but it actually calmed me down a bit.

Everything else is looking good. There was nothing concerning from the anatomy scan and no indicators of TTTS. I found a new doctor and have an appointment scheduled for Tuesday. I'll keep you all posted on how things go.

Hi! I haven't been on the boards in forever :( But I had mono/di twin boys in July 2012.

Everyone has pretty much already said it, but I wanted to reiterate how important it is to get that protein and extra calories in! I know it's hard as you get bigger, but it's soooo important when the babies share a placenta to EAT! :) I'm more of a 'crunchy' natural mama :wink: , so it was really hard having all the monitoring; but in the end it was worth it. Just make sure you really go for what you need and want. The MFM's told me the whole time there was a super high chance of having a C-section and they scared me sh$%tless the entire pregnancy. But thank god, it all worked out and I got the birth I wanted. So good luck and Congrats!!! It's an amazing ride; twins are awesome!!!!

I wanted to give everyone a quick update. I met with my new OB last week and it went really well. We talked for a while about the transition from midwifery care to OB care and he's on board with making this as hands-off as possible. Of course I still have to have appointments and monitoring every two weeks but at least I feel like he's really taking the time to listen to what I want. I go back on the 25th for my next appointment.

Otherwise my DH and I are in a much better place about having twins. The initial shock has worn off and we're settling into our new reality... we have more excited moments than "oh crap" moments now. :) Thanks for all your support! I'm looking forward to sharing this crazy experience with all of you.

Sounds like you have a really good attitude though and that will get you a long way once the going gets tough.
